Download and print the Information Packet below
Submit your Information Sheet: you can either
complete the first page of the Information Packet (below), OR
fill in the Google Information Form
Submit, by email, all required docs in the timeframe indicated
Information Form (via the Google Doc or the packet below), AND
the documents listed on page one of the packet
After I have your Information Form and copies of your documents, your date will be confirmed and I will assign your cross country scenario.
If I do not have these items in the required time frame, you could forfeit your reservation!!
